Once you have completed the online portion of the registration, please email the following documents to tiffany.page@jordandistrict.org (a hard copy of the forms may also be brought into the office):
- Original birth certificate
- Parent/legal custodial guardian's picture identification
- Current Immunization Record
- Proof of residency
